They include persistent fever, bleeding into the skin, pallor, headaches, vomiting, swelling of the neck glands, generalized distention of the abdomen, swelling and constant pain in the bones and joints etc. All of these could be because of a common disease or condition. Moreover, cancer in children is rare. Therefore, parents should not get worried if their child suffers from these symptoms. Only if the symptoms are present for a long time despite the initial treatment can we suspect cancer. Parents should have a discussion with their child’s doctor if they have any concerns regarding their child’s health.

What tests may your child have to undergo at the time of diagnosis and treatment of cancer?

If cancer is suspected, specialist doctors may need to do further tests to understand the type of cancer your child has. These are called diagnostic tests. They may also conduct several tests to understand whether your child’s cancer has spread to other parts of their body. These tests are called staging investigations. Treating doctors will conduct tests to assess your child’s general health and see if their body is fit to receive chemotherapy. They may perform additional tests during the treatment to understand how it affects your child’s body and repeat them post-treatment to assess how the cancer in your child responds.

Depending on the type of cancer suspected, the doctor will decide which tests and at what stage they should be conducted.

Here’s a list of standard tests performed during cancer diagnosis and treatment. Some reasons for a blood test include:

Detecting cancer

Checking for infection

Monitoring how well the treatment is working

Observing the side effects of the treatment

Making sure your child’s body’s systems (such as the liver or kidneys) are still working properly

Matching your child’s blood with a donor’s if your child needs to receive blood

Monitoring your child’s blood count (the number of red and white blood cells and platelets in their blood).

Bone marrow test.

A bone marrow test looks at bone marrow samples under a microscope to see if a child has blood cancer or cancer in any other part of the body has spread to the bone marrow.

Bone marrow is a soft tissue that lies inside the bones, and it is where all the blood cells are produced. In a bone marrow test, the doctor inserts a needle into the bone to collect some bone marrow. It is done in children on the back of their hip bone after making them sleep with sleep medicine (General Anesthesia) for a short period to avoid pain. Therefore, they will not have any pain during the procedure.

Lumbar puncture

This test is used to examine the fluid around the spinal cord (called the CerebroSpinal Fluid or CSF) to see if the CSF contains cancer cells. In a lumbar puncture, the doctor inserts a needle between the bones in the lower spine and takes a few drops of the CSF to examine under a microscope. This test is done in children usually after making them sleep with sleep medicine ( General Anesthesia) for a short period. Hence, they will not have any pain during the procedure.

Doctors also use this kind of procedure to give cancer medicine (chemotherapy) to patients with some cancers like leukemia. It is called intrathecal chemotherapy administration (IT). Sometimes, children may have a headache after the procedure, so your child may be asked to lie down flat for a while once they have had the test.

Biopsy

Biopsy means removing a tiny piece of the cancer suspecting part of the body, either with a specially designed needle (needle biopsy) or by a minor operation (open biopsy). The sample is then examined under microscopy to determine what type of cancer it is.

Your child may also undergo CT (Computerized Tomography), MRI (magnetic resonance imaging), or an Ultrasound scan if required. These are tests used to take detailed photos of internal body parts. They are used to diagnose cancers, evaluate spread of cancers and to assess the treatment responses.

What are the treatments of cancers?

Chemotherapy, radiotherapy and surgery are the primary treatments for all cancers. Depending on the type of cancer, stage of cancer, and general health of your child, treatment of your child’s cancer may include just one of these treatments or a combination of the three. There is a separate chapter for chemotherapy which explains what they are, their side effects, and measures to reduce complications. Before the doctor puts your child’s treatment plan into action, try to understand what exactly will happen, including the benefits, risks, and potential side-effects of the treatments the doctor has offered you.

Most of the signs and symptoms seen in children’s cancer are similar to common childhood diseases.
