What is leukaemia?

Leukaemia, also known as blood cancer, is a cancer of the white blood cells (WBC) in the blood. 30-35% of all childhood cancers are Leukemia. There are four types of blood cancers that commonly occur. Among these, we see three in childhood: Acute Lymphoblastic Leukemia (ALL), Acute myeloid leukemia (AML) and Chronic Myeloid Leukemia (CML)

2. What are the blood cells?

There are three types of blood cells.

Red blood cells (RBC), which carry oxygen around the body.

Platelets, which help the blood clot and control bleeding.

White blood cells (WBC), which help to fight infection.

3. What is Bone Marrow?

Bone marrow is the spongy substance situated inside some bones in the body. All blood cells are made in the bone marrow.

4. How does Leukemia develop?

Usually, white blood cells develop in an orderly and controlled fashion in the bone marrow. However, in leukemia, the process gets out of control, and the cells continue to divide without proper maturation. These immature dividing cells are the cancer cells and are called Blast cells.

5. What are the signs and symptoms of Acute Leukemia?

As the cancer cells of leukemia continue to multiply in the bone marrow, the production of normal blood cells is reduced. When red blood cells are reduced, this leads to anemia, which can cause tiredness, lethargy, and pale skin. When the platelet count is reduced, it causes bleeding and skin bruises. Similarly, if white blood cell production is reduced, children get frequent infections leading to persistent fever.

At one stage, leukemia cells or blast cells, invade body organs (like the lymph nodes, liver and the spleen) and cause enlargement of these organs. During this time, the child is likely to feel unwell and may complain of aches and pains in the limbs.

At first, the symptoms are just like those of a viral infection, but when they continue for more than a week or two, the diagnosis becomes clear.

6. What are the causes of leukemia?

The exact cause of leukemia is unknown, and research is ongoing to unravel the possible causes of this disease. However, like all cancers, leukemia is not infectious and cannot be passed on to other people.

7. How is leukemia diagnosed?

We need to evaluate a bone marrow sample to confirm a leukemia diagnosis, and check whether cancer cells (blast cells) are present in it. We also send the bone marrow sample to the genetics lab to look for any abnormal chromosomes. These tests will pinpoint the type of leukemia and help doctors decide on the best treatment for it.

8. How do I know what stage of leukemia my child is in?

In Leukemia, we do not use the term stage to understand the spread, as in other cancers. However, leukemia can be grouped into different risk groups such as average risk, standard risk, high risk and very high risk.

9. How do doctors classify children with leukemia into different risk groups?

There are many factors that are being considered when placing children with leukemia in different groups. They include: white blood count (WBC), age of the child at the time of diagnosis, genetic variations seen in cancer cells, type of leukemia, presence of cancer cells in the brain or fluid around brain (CSF CerebroSpinal Fluid), response to treatment given in the first month of diagnosis as tested by MRD (Minimal Residual Disease). We can also perform a lumbar puncture test to see if the spinal fluid contains any leukemia cells (blast). Depending on the risk group, doctors will decide your child needs strong treatment or mild treatment.

10. What is MRD (Minimal Residual Disease) ?

This is a special test to assess the remaining of the Leukemia after initial treatment. This is performed at different time point of the treatment. But most commonly done after one month of treatment. Depends on the value of the MRD, doctors decide further treatment.

11. What are the treatment options for leukemia?

Chemotherapy is the primary treatment option for leukemia and is given according to a treatment plan, also called a protocol. The type of chemotherapeutic agent given, the quantity of the medicine, and method of giving the medicine are all different in different protocols. The treatment protocol is slightly different in each country, depending on the medication available and the facility set up. Treatment for leukemia aims to destroy the leukemia cells and enable the bone marrow to work normally again. Treatment depends on what are risk factors present in your child and MRD results.

12. What is chemotherapy?

Chemotherapy, or in short “chemo” means using medicines (drugs) called chemotherapeutic agents to treat cancers.

13. What is the role of a bone marrow transplant, cart T cell and immunotherapy in leukaemia treatment?

Most leukemias can be treated using only chemotherapeutic medicine without resorting to these treatment options. However, doctors use these modes of treatment only in cases where the leukemia is likely to come back or has come back (recurred) in the past, even after giving standard chemotherapy.

14. What is the duration of leukemia treatment?

Duration of leukemia treatment is dependent on the type of leukemia and the initial spread of leukemia. Initial treatment is very intensive and is usually given after admitting the patient in a hospital. Some leukemia treatments can last up to 3 years.

15. Follow-up care for Leukemia

Many children with leukemia get cured. In addition, most children with leukemia grow and develop normally later in life. However, if your child had leukemia in the past, make sure you visit your doctor regularly for a few years, even after proper treatment has been done.

If you have specific concerns about your child’s condition and treatment, it is best to discuss them with their doctor as they know your child’s situation in detail. Your child’s doctor will be able to provide the exact treatment details.

1. What is leukaemia?
