They can affect children of any age.

Different types of brain tumors

There are different types of brain tumors. The main types are Astrocytoma, Ependymoma, Glioma, Meningioma, Pinealoma, Craniopharyngioma and medulloblastoma, but there are many other, less common types.

What are the typical symptoms of childhood brain tumors?

These depend on the size of the tumor and which part of the brain is affected.

Usual symptoms and signs are:

Repeated and persistent headaches (often worse in the morning)

Continuous vomiting (usually in the morning) or feeling sick

Walking difficulties or poor coordination during normal activities

Fits (seizures) or tremors

Feeling irritated or losing interest in day-to-day things

Changes in behavior

Abnormal eye movements, blurring or double vision

Some of these symptoms are prevalent even without a brain tumor, which can confuse most people in the early stages. This can cause a delay in a brain tumor diagnosis in children.

What are the investigations we need to diagnose brain tumors? CT or MRI scan?

We can diagnose most brain tumors by a CT or MRI scan. CT scan is quick and is often helpful as a first-line investigation, but it does not give as detailed a picture as an MRI. An MRI scan gives a more detailed picture, but it takes much longer to do. The machine is very noisy, and sometimes, we need to use sleeping medicine to put the child to sleep before doing the MRI.

Biopsy

A biopsy is a procedure where a tumor sample is removed, and we look at the brain cells under a microscope. It helps us understand what type of tumor is active. It is usually done through a minor operation.

Treatment

Treatments for brain tumors include surgery, radiotherapy, and chemotherapy. As there are different types of brain tumors, treatment is not the same for everyone.

First, a detailed examination of the tumor cells will confirm the diagnosis. Then, a multi-disciplinary team of specialists, consisting of a pediatric oncologist, radiation oncologist, neurosurgeon, pathologist, and radiologist, decides the final treatment. The child may need surgery alone or a combination of surgery, followed by radiation and chemotherapy. The specialists also consider the age of the child when they decide on the treatment.

Surgery

Usually, a neurosurgeon will operate to eliminate as much of the tumor as possible. Operations can go on for very long.

Sometimes, the fluid in the brain will not circulate freely because of the tumor or brain swelling. Then, the surgeons have to place a fine tube (shunt) to drain the excess fluid from the brain and into the abdominal (tummy) lining. This is called a VP shunt.

After the operation, the child may spend some time in an intensive care unit so that the nurses and doctors can watch them closely.

Rarely, because of the tumor’s position in the brain , biopsy and surgical resection may not be possible. Doctors may reach the diagnosis based on the appearance on scans, doctors may decide to proceed with other treatments like radiation and chemotherapy without a biopsy or total surgery.

Radiotherapy

Radiotherapy treats cancer by using high-energy radiation beams. As the beam passes through the head, it gives up energy, destroying tumor cells inside the head. Radiotherapy aims to kill as many abnormal cancer cells as possible. However, while radiotherapy kills the cancer cells, it can cause damage to surrounding normal cells as well. Therefore, radiotherapy can cause short term and long-term side effects on the child. The side effects of radiotherapy depend on the child’s age, dose and duration of radiotherapy and area of the brain where radiotherapy is given.

Radiotherapy is delivered in several daily doses for up to 6 weeks, but each dose usually takes a minute or two and is absolutely painless.

Chemotherapy

Chemotherapy agents are medicines used to destroy cancer cells. Doctors give chemotherapy into the blood vessels (veins), or sometimes as a liquid or tablet by mouth. It may occasionally result in side effects, which can be short term or long term. Side effects of chemotherapy depend on the type of medicine, dose and duration of the treatment.

Doctors also use steroids and anticonvulsant medicine in children with brain tumors

Steroids

These are anti-inflammatory medicines that reduce swelling in the brain.

Anticonvulsants

These medicines help prevent fits, which can be a problem before or after the operations. They may be necessary for a short period but are occasionally needed for longer.

Follow-up care

After treatment is over, you should continue to see a cancer doctor regularly. This is to check how well your child is recovering from the treatment. It also monitors your child’s health and ensures the tumor does not come back. Regular follow up is recommended to detect any long-term complications from the cancer treatment such as hormone deficiency and learning issues so that doctors can support your child further.

Brain tumor in children, What parents need to know.
