What is liver cancer in children?

It is cancer affecting the liver of children. The two types of liver cancers that occur are:

Hepatoblastoma – is a common type of liver cancer and usually occurs in children under five.

Hepatocellular carcinoma (HCC) – is rare and usually occurs in older children.

2. Where is the liver in the body?

It is present in the upper part of the abdomen. The lower ribs surround and protect it from injury. A tube called the bile duct joins the liver to the small intestine (duodenum). This duct transports the bile produced by the liver to the intestine.

3. What are the causes of liver cancer?

We do not know the cause of hepatoblastoma in children. However, vaccination of infants against the Hepatitis B Virus (HBV) reduces the risk of developing Hepatocellular cancer in children.

4. What are the signs and symptoms of Liver cancer?

The most common symptom is a painful lump or swelling in the abdomen. Other frequently seen symptoms include weight loss, loss of appetite, nausea (feeling sick) and vomiting (being sick).

5. How is liver cancer diagnosed?

We need several investigations to diagnose liver cancer.

The most practical test to check for cancer in the abdomen area, including liver cancer, is the abdominal ultrasound scan.

If there is any suspicion, a test to measure the level of alpha-fetoprotein (AFP) can help confirm the diagnosis of liver cancer.

The most important test to verify a liver cancer diagnosis is by doing a biopsy.

6. What does a liver biopsy mean?

In a liver biopsy, we remove a tiny piece of the liver and look at its cells under a microscope. It is a minor operation, which we will do under general anaesthesia.

7. What does staging investigations mean?

Once the liver cancer is confirmed, further tests need to be done to find the original cancer site and the extent of cancer within the liver and outside the liver. These tests are known as staging investigations. They include blood tests, X-rays, CT, or MRI scans.

8. What tests need to be done with liver cancer?

A special test to measure the alpha-fetoprotein (AFP) levels is quite helpful in liver cancer diagnosis. This particular test can also assess if the cancer is responding to the treatment or not. Additionally, it may help determine if the liver cancer has come back after the treatment. Therefore, AFP is also known as a liver cancer marker.

9. What are the treatments for Liver cancer?

Treatment for hepatoblastoma and hepatocellular carcinoma is different.

Treatment for hepatoblastoma includes chemotherapy and surgery.

Chemotherapy is given first, depending on the liver cancer staging. It aims to destroy the cancerous liver cells and any other cancer cells that have spread to other parts of the body.

Surgery is the next step, to remove any remaining cancer cells. It is done a few weeks after chemotherapy. We usually give another round of chemotherapy after surgery as well. Your child’s doctor will provide you with more detailed information about the treatments and any side effects you may experience.

For hepatocellular carcinoma, the treatment plan is slightly different. The role of chemotherapy is less definite, and surgery is the primary treatment option. Your child’s doctor will explain better about the treatments and their complications.

10. Is liver transplantation necessary in children with liver cancer?

It depends on the extent of the liver cancer. If the cancer has spread to most of the liver, a liver transplant will most likely be necessary.

11. What is the treatment plan if the liver cancer comes back?

Over 75% of children with hepatoblastoma recover completely, but the outcome for hepatocellular carcinoma is not as reliable. If the cancer returns after the first treatment, we call this a recurrence. It can come back in the liver or any other part of the body. When it does, alpha-fetoprotein (AFP) levels in the child’s blood will rise, even before anything is seen on the scans. However, there are treatments available if that is the case.

12. What kind of follow up is needed after treatment?

When the treatment ends, your child will have regular blood tests to measure AFP levels in the blood (if required), along with scans and chest x-rays. Over 75% of children with hepatoblastoma recover entirely after treatment. Unfortunately, we cannot say the same for hepatocellular carcinoma. The outcome hinges on person to person.

13. What are the outcomes for children with liver cancer?

The cure rate for Liver cancer is very high, especially for hepatoblastoma. However, some children may develop side effects after treatment. It depends on the type of treatment your child received. When your child completes the treatment, you should take them for follow-up visits regularly. There will be occasional scans, x-rays, and blood tests done for a few years after completing treatment.

1. What is liver cancer in children?
